    I filled out the survey but unless I’m still asleep this morning, there was no request for my contact information so that you could send me the eBook mentioned.

    My email address is included as part of this comment. However, it seems most reasonable to include the request for that information within the survey.

    The link to download the eBook is available on the final page of the survey, after you press submit.

    We do not ask for any personal information in the survey because it is an anonymous survey.
