Need help with some business forms for the State of Michigan

I would like to know if anyone here is familiar with the form UIA1020?  It has to do with unemployment taxes.  I have to do all of my own accounting and I have very little experience.  My dad has colon cancer and he never paid the accountant so my accountant will not fill out any forms for me.  I am in college to learn all of this stuff, but these reports are due next week.  I have lots of business problems but I am sure no one wants to hear about them.  Any replies would be greatly appreciated!

Hey:

I wish I could help but I am from NJ and not an accountant but I did get a number that supposedly will offer you some help.

Now do you have the gross monthly wages? And the number of part time and full time employees? It looks like the form wants the monthly count for each month in the quarter.

Now I understand what its use is and what information its asking for but the question is, do you have this information?
