My opinion on this:

1) Title tag should definitely contain your target keyphrase

2) h1 tag at top of page should also contain the keyphrase

3) h2/h3 tags can break down subsections of page with a contextually relevant variation of the keyphrase

4) Placement of relevant images will allow you to include keyword relevant alt tags that also comply with equal access guidelines.

5) A couple bolded sentences on your page that include relevant words may be a good idea.

6) Keywords in urls / domain names / folder names / page names will be helpful.

7) Keywords in the meta description should make you page more relevant to some search results (some search engines actually display the meta description in search results)

8) ** The overall keyword density of your page should be "reasonable". Do not insert too many keywords, or the page may be penalized as "keyword spam". You might try using the keyword density tool on my website.

The position of a keyword plays a very important roll for your website. Many people are known to use it in places that are rarely seen by people and the search engine spiders. When you are considering your keyword ensure that it matches the content of your website and that you do not actually over use it.

Many people are known to keyword stuff. DO NOT DO THIS! Keyword stuffing can actually get you pinged as a spam site and removed from the SERPs. This is something you want to avoid. Keywords are placed to get you seen in a certain market, so match your market, place it in your content, your title if it fits, and in some instances, your domain name.

A keyword is your entrance to your website. Make it something people will search for and find you. Review your competition and place your keyword throughout your website. Just remember, it doesnt have to be everywhere, just in important places.

yes, the higher the better and also include h1 and bold tags

i also put keywords at the end of the page so the search engine will see that the content is still relevant to the keyword
