There are alot of books available that provide the forms.  One in particular is "The Inventors Bible" by Docie, Sr.  You may also want to consider joining an inventors network, i.e. Invent Right.  They provide forms, advice and opportunities to participate in teleseminars partnered with Inventors Alliance.  I am a member of Invent Right and love the support and information that they give to all their members.  In addition, you`d be paying alot more for lawyers compared to the cost of joining this organization.  I hope this was helpful.  Visit their website: www.inventright.com

Good Luck !
