By definition, a consultant provides advise for a fee. An alarm consultant for instance might provide advise on installing an alarm system. But, he would not install the alarm system.
A contractor provides a service or performs a specific piece of work for a fee. An "Independent contractor" controls the means and method of doing the work.
You also have to be careful how you structure your relationship with a contract employee. The more control you exercise over the when, where and how they do their job; the closer you come to being their employer.
You also don`t need and attorney. I have used a paralegal in the past for a lot of my legal issues.
Micro Soft and Google (Google Docs) have web sites where you can down load a variety of offices forms. You might start your search for a form there. Another place to go is a local paralegal service. You tell them what you want and they can draft the form for you.
