For a dating website, what elements(expenses, profits, etc)  do I need to worry about for my 5 year forecast?

Web start up costs: hosting, web development, domain name.
On going costs: hosting, domain name renewal, possible website maintenance.
Marketing/Promotion costs: online ads / pay per click, other marketing (print/radio/video).
May be affiliate costs - if you are affiliated with another site.

The purpose of a forecasting report is for you to `predict` your figures. Since you`re just starting, the best way to get information which you can use for generating figures is from competitors. However, since some of these information might be confidential, you might find difficulties with the access.
 
But since you`ve chosen this type of business, you should have some idea of what sort of revenue you could earn, plus the expenses you need to shoulder - there are operating expenses that are normal and regular, and there are capital expenditures which require some funding, which you can then allocate periodically.
 
If you can project/predict figures for your Revenues and expenses, then you already can accomplish step 1.
