I am really stumped on this one. We are manufacturing and distributing a
Heavy Duty Lotion. We are in the process of creating sample packs that will
hold 2 ml of product. My question is, does anyone know what needs to be
included on this pack by law? The printer gave me a link to the FDA site and
I have gone through most of it and found nothing that referenced what to do
with a sample pack. I have many competitors sample packs and none of
them list the ingredients, but I would like to be sure on this. Any help would
be great!!!

You might check to see if the Cosmetic, Toiletry, and Fragrance Association can be of any help.  The CTFA is also a sponsor of the Cosmetic Ingredient Review.   The may be able to provide you some answers - or at least other resources - in regards to your labeling questions.

Many years ago, when I was working on a label for a product sample - in another industry - we did not need to list ingredients on the sample because it was being specifically produced to be given away, rather than sold.

Thanks for the info. Your final comments have prompted a new question. Is
selling the sample pack the deciding factor? Our packs will also be free.
Have you found that in those cases the ingredients do not have to be
included?

In packaging for other product freebies - especially those still under some development - I have not been required to list all the ingredients until the product was on the market for sale.  I don`t know if that varies from state to state.  Perhaps one of the resources I included can clarify things for you.

One of the few things I believe the law requires on sample packs is if there is allergetic alerts (such as "this product may contain or be manufactured around peanuts"), so I would show care in those reguards, but beyond that, there is no "Rock solid" list required to my knowledge.

I would list the top one or two indgredients (Items that are 40% or greater), and allergy warnings (if needed) just to be on the safe side.
